Scheduling Assessments

If you would like to schedule a Chemical Use Assessment (also called a Rule 25), call 763-689-1711 and ask to schedule a Rule 25. The person setting your appointment will ask you certain questions to determine if you are eligible to have the assessment through Isanti County. A Rule 25 Assessment can only be completed for persons who are residents of Isanti County and meet eligibility and income guidelines.

On the Date of the Assessment

Please be on time for your assessment. If you are going to be more than 15 minutes late for the assessment call 763-689-1711 to be sure the assessor can still see you. If you are more than 15 minutes late for your assessment without calling, you will have to reschedule your meeting for the next available opening.

Because the Rule 25 requires collateral information, you should bring the names, phone numbers and addresses of at least two people that know you, who can give information to the assessor regarding your chemical use. If someone has told you to get the assessment, like a probation officer, this should be one of the collateral contacts. The assessment can take from one to one and a half hours to complete. The assessor will meet with you and ask questions about the history of your chemical or alcohol use, current use, and how it is affecting your daily life and those around you.

When the assessment is completed, if treatment is recommended, you and the assessor will arrange for a referral to the program that best fits your needs. It is important to remember that openings in treatment programs are not always immediately available and it can take from several days to several weeks to get into a treatment program.

Confidentiality

The information you give in the assessment is regulated by federal law and is strictly confidential. You must sign a release of information consent before information can be shared with anyone outside of the chemical and mental health unit at Family Services.
