Aging & Disabilities

The Isanti County Aging and Disabilities Unit assists people of all ages and abilities find resources and services to live safely and independently in their own homes and community. Services are provided through assessment and case management.

MnCHOICES Assessment

MnCHOICES is a comprehensive assessment that captures a person’s goals, assessed needs and preferences. The assessment is provided by Social Workers and Public Health Nurses who are specially trained. MnCHOICES determines eligibility for Long Term Services and Support programs.

Case Management

The Aging and Disabilities Unit provides ongoing case management to eligible people with disabilities with the goal of helping people live independently in the community. The role of case management is to develop a support plan based on the person’s needs, refer to service providers or resources in the community, coordinate those services and monitor the person’s ongoing needs.

Case management is provided through Home and Community Based Waiver programs. Each Waiver program offers a variety of services.
