Developmental Disability/Mental Retardation/Related Conditions Isanti County Family Services
provides case management (care coordination) for children and adults with mental
retardation or related conditions (commonly known together as “developmental
disabilities”). The case manager assists persons with developmental
disabilities to gain access to needed supports and services. To qualify for county case management services, persons need to meet specific criteria in order to be eligible. A summary of the criteria is listed below: A diagnosis of mental retardation: Includes an IQ of 70 or less and demonstrated deficits in adaptive behavior that manifests before a person’s 22nd birthday. A diagnosis of a related condition: Has a severe, chronic disability related to cerebral palsy, epilepsy, autism, etc., found to be closely related to mental retardation because the condition results in impairment of general intellectual functioning or adaptive behavior similar to that of persons with mental retardation that manifests before age 22 and results in substantial functional limitations. Some persons with developmental disabilities may be eligible for a program known as the Developmental Disabilities (DD) waiver. This program is targeted to persons who require a level of care in the community comparable with institutional placement, are certified disabled, and are eligible for Medical Assistance. Authority: Minnesota Statute 245B, Services to the Mentally Retarded Other community professionals can refer children for DD case management services by discussing the proposed referral with the child’s parent(s)/guardian, obtain a signed consent for release of information. Send a copy of the signed release form and a written statement that contains identifying information, recognized concerns, psychological evaluation with adaptive behavior skills assessment completed with in the past 365 days (if available), and any other pertinent information to: Oakview Office Complex 763-689-1711 Developmental Disabilities Eligibility Assessment: · Upon receipt of the referral or contact from the parent, intake information will be sent to the parent. Information will be collected about diagnosis and eligibility. · The social worker will provide a HIPPA notice, complete an application for services, collect other appropriate eligibility information. · If the materials fail to document the need for or eligibility for DD case management services, the family will be notified in writing of the denial of services, citing the statute and rules that pertain to the requested services. Consumer Support Grant (CSG): Program that provides a cash grant in place of a “fee for service” home care program. Must have Medical Assistance, eligible to receive home care services and direct and purchase your own care. Assessment is completed by Isanti County Public Health Nursing Services at 763-689-4071. DD Waiver Program:
Must be on MA or TEFRA to be considered. Eligibility for this program is
determined by Isanti County DD and Family Services staff. Family Support Grant (FSG): Program that provides a cash grant to families of eligible children with a goal of preventing or delaying an out of home placement and promoting family health. Child must have a certification of disability. The maximum grant is $250 per month and there is an annually adjusted income ceiling to qualify. Group Residential Housing (GRH): Funding for room and board based on income and financial eligibility. Medical
Assistance:
A federal program that pays for medical
care and some support services for low income families and individuals 18 years
or older. Medical Assistance-Employed Persons with Disabilities (MA-EPD): For people with disabilities who work at little or no cost. Contact a financial worker at 763-689-1711. Private
Health Insurance:
May pay for assessments, adaptive equipment, therapy, counseling, and support
services Supplemental Security Income (SSI) and Retirement & Survivors Disability Insurance (RSDI): Cash benefits payable to children with qualifying disabilities whose families have very limited income. At age 18, family income is no longer considered for eligibility. Tax
Equity and Financial Responsibility Act of 1982 (TEFRA): A program that provides
medical assistance for children with qualifying disabilities who live with their
families and do not qualify for medical assistance because the family's income
is too high. There is a parental fee based on family income and family
