HEALTH RULE 25 ASSESSMENTS (CHEMICAL USE ASSESSMENTS) Scheduling Assessments: On the date of the Assessment: Because the Rule 25 requires collateral information, you should bring the names, phone numbers and addresses of at least two people that know you, who can give information to the assessor regarding your chemical use. If someone has told you to get the assessment, like a probation officer, this should be one of the collateral contacts. The assessment can take from one to one and a half hours to complete. The assessor will meet with you and ask questions about the history of your chemical or alcohol use, current use, and how it is affecting your daily life and those around you. When the assessment is completed, if treatment is recommended, you and the assessor will arrange for a referral to the program that best fits your needs. It is important to remember that openings in treatment programs are not always immediately available and it can take from several days to several weeks to get into a treatment program. The information you give in the assessment is regulated by federal law and is strictly confidential. You must sign a release of information consent before information can be shared with anyone outside of the chemical and mental health unit at Family Services.
